Skin quality is one of the most important contributors to your appearance. A good skin care program is integral to the best appearance of your skin. Skin types vary and we will occasionally deviate from this program to customize your skin care. However, this article is meant to familiarize you with the importance of what we consider to be a basic program.
Good skin care starts with overall good health and
nutrition. You must be careful in the sun and be religious with the use of
sun block. This is both for the appearance and health of your skin. You must prevent skin cancer if you are not to have scars indicative of its removal. Avoiding fatty foods such as chocolate will reduce the incidence of blemishes. Drinking plenty of water (at least 6 glasses per day) will keep your skin well hydrated, especially during the winter months when the house air is so dry from the heating systems. Avoiding areas in which people are smoking will help keep your skin healthy. If you are a smoker yourself, stop reading here. Quitting smoking will give the most benefit to the skin and the other treatments will help only minimally if you do not quit.
There are a number of skin care products integral to a good program. We will try to explain the benefits of each and how to use them. Introduce only one of the products to your skin at a time. Allow at least two weeks to get used to a product before adding a new one.
Alpha Hydroxy Acids (AHA’s) are products which cause desquamation or sloughing of the dead skin cells on the outer layer of the skin. This works in conjunction with Renova because when your skin cells turn over faster, there is a quicker buildup (at least initially) of dead skin cells which is the cause of the flakiness. There are many products which contain AHA’s. Most of the creams available in your local pharmacy do not have adequate strength to do the job. In this office, we recommend The MD Formulations line of products. They are the best we have found. You should start on the cleanser first. Other products may be added as necessary. This can be used morning and night as the first treatment to prepare your skin for the other products.
Antioxidant formulations are important. These increase collagen synthesis creating a better appearance in the skin. They also help prevent ultraviolet damage to the skin and prevent photoaging. For this reason, we ask you to use these products in the morning. Remember, the Renova is used at night. Vitamin C products are the first prescribed elements of the program. We use Active-C products from LaRoche-Posay. These come in two strengths, a weaker one for the delicate eyelid area and a stronger one for the rest of the face. There are also two different formulations. One is a cream based product for drier skin and the other is a serum for more oily skin types. You will need both the eye and face products. The facial product is too strong for the eyes and the eyelid product is not strong enough for the face. Prevage™ (idebenone 1%) is a new antioxidant product from Allergan, the makers of Botox®. Although more expensive, it is one of the most potent and effective antioxidant compounds. It helps protect against fine lines and wrinkles and improves the hydration of the skin.
Vitamin A products increase the cell turnover rate (makes skin heal faster), reduces photoaging, and reduces the degradation of collagen. Collagen is one of the most important components of the skin and gives it the structure and full-bodied appearance. It initially was available as Retin-A for the reduction of acne. The Renova formulation is a gentler base that fewer people are sensitive to and is an expensive prescription product. We recommend Retinol Cream from Biomedic. It is used once per day, generally after cleansing the skin. It should be used at night since it can make the skin more sensitive to the sun. It occasionally causes flaking and dryness of the skin when you initially start using it, but this generally settles down after a few weeks. Depending how sensitive your skin is, a number 30 or 60 (stronger) strength will be recommended.
Moisturizer is always an important part of good skin care. We have found an excellent product that is very hypoallergenic and extremely comforting to the skin. Our Hydrating Cream Concentrate is the product most of our patients rave about. It can be used both morning and night. It should be applied after the Renova or Vitamin C has soaked into the skin for few minutes. Remember also to drink plenty of water during the day.
Sunblock is important to use at all times when in the sun. MD Forte makes excellent sunblocks that are non-irritating and very hypoallergenic. There is an excellent SPF 20 in a cream base that is good for everyday use. They also have another formulation (Aftercare Environmental Protection Cream) which is important for patients having microdermabrasion or who have just had surgery. It is a disappearing zinc oxide base and has an SPF of 30. It is very waterproof and also protects the skin well from windburn.
Microdermabrasion (Megapeel) is the most natural way to evenly and safely exfoliate your skin. Fine crystals are used to gently blast away the dead skin cells. This debris is then all vacuumed up in the same nozzle. Vacuum pressure can be increased to evenly penetrate deeper skin layers. These treatments are available in some salons but the physician based machines are much more powerful and can create a greater improvement. A series of Megapeels can create a healthy appearance in dull, lifeless skin. They can also improve fine lines and mild pigmentation problems from photoaging. They may also be effective for some scars and for acne. After the treatment, redness may last for one half hour up to a full day depending on the treatment. The other major benefit of microdermabrasion is that it allows your other products to better penetrate the skin after the outer cells have been removed. It improves the synergistic skin care program.
IPL (Intense Pulse Light) therapy is another new and exciting process to improve the appearance. An extension of laser science, this machine creates many of the effects of lasers without the pain and healing process involved with many lasers. Permanent Hair Removal is performed over a series of visits, can be done anywhere on the body and can even be performed on tanned skin unlike the lasers. Rosacea and fine blood vessels (telangiectasias) of the face can easily and effectively be removed. IPL is perhaps the best treatment for Rosacea. Pigment spots of the face can be removed. The skin of the entire face can improved with a series of Photofacials. We use the Palomar MediLux™ IPL machine.
